Access Hospice Care Team

Access Hospice provides care using an interdisciplinary team approach to address each individual's physical, psychological, interpersonal, and spiritual needs.

To promote this philosophy, the Hospice interdisciplinary team supports existing patient/family coping mechanisms or provides guidance in developing alternative methods of coping.

Patient pain and symptom management is emphasized. The team offers support for the caregiver and family both during the dying process and after death has occurred.

Medical Director

Our Medical Director is a well established physician in our community. He receives regular updates and information to best address client needs and regularly coordinates with the primary care provider to ensure the best care.

Hospice Nurses

Our nurses are trained to assess and bring care to the patient, communicate with doctors, educate family members, and administer care treatments to help the patient to be more comfortable.

Chaplain

Our Chaplain is an ordained minister with special training in counseling. He is sensitive to each person's unique spiritual journey. He works with any spiritual guide or can provide one-on-one support.

Social Worker

Our Social Worker is trained and licensed in Missouri. During this time of need, knowledge of state and local resources is invaluable.

Bereavement Counselor

Our Counselor is a licensed clinician with years of experience. She specializes in helping the patient and family through this difficult journey.

Hospice Aide

Our Hospice Aides add a personal touch in our patients daily life; whether it's bathing, personal care, light housekeeping, shopping or errands.

Volunteers

Our Volunteers are caring, devoted individuals worth their weight in gold. From friendly visits, preparing meals or helping with correspondence, they make all the difference.
